La Vida Real's Enliven™ Memory Care offers personalized care within a secure residential environment. Our carefully selected and trained staff works closely with memory impaired residents and their families, developing individualized and innovative programs that focus on meaningful tasks that are engaging and stimulating while being stress-free for the resident. Here are the services provided in our memory care residences:

• Three meals daily, including snacks
• Private and semi-private studio residences
• Secured, residential environment
• Weekly housekeeping, personal and flat linen laundry service
• 24-hour staffing
• Interior and outdoor walking paths
• Specially designed common areas and outdoor courtyard
• Full calendar of social, cultural, educational and recreational activities

Rehabilitation Services

Through our partnership with ONR, residents at La Vida Real are fully equipped to start to heal and feel better. Rehabilitative services can be tailored to the unique needs of each person to support them in regaining or retaining health and independence.

• Physical Therapy to help you bounce back and feel better as soon as possible
• Occupational Therapy using sensorimotor treatments to increase strength, endurance, range of motion, coordination and balance
• Speech Pathologists are ready to help with speech therapy or swallowing disorders by including exercises to build up oral muscle strength and function
• Available Respiratory Therapists offer detailed treatment options designed to return residents to their optimal breathing capacity

Concierge Medicine

We know that offering our residents faster, more convenient access to healthcare services allows them more positive results and provides peace of mind for family and friends. That’s why, here at La Vida Real, residents have access to concierge medical services delivered in the privacy of their residence by Pine Park Health.

By offering a variety of integrated healthcare services, our goal is to address the unique needs of our residents, many who may find it challenging to find specialists, make appointments or go to the doctor's office and ultimately improve health outcomes through expanded access to services.  

Concierge Medical Services Can Provide:

• A team approach to care
• Convenient, onsite healthcare services
• Regularly scheduled visits in the community by the MD/NP team alleviating the need for transportation
• Care coordination with the MD/NP/Pharmacist/Behavioral Health in coordination with the community’s care team
• Regular communication with the resident’s family
