Senior Resource Group, Inc. (SRG), which develops, owns and operates service-enriched senior living communities across several states, is introducing a new program called SRG Assure™. The program, featured in McKnight's Senior Living, focuses on leveraging SRG’s more than 30 years of experience in senior living to help residents receiving supportive care services transition back to the community after a hospital stay and lessen the risk of being readmitted back to the hospital.
“Being discharged from the hospital doesn’t always mean a patient has fully recovered. In many cases, they haven’t, which can make that transition back to normal life challenging,” Isaac Hagerman, vice president of health and quality at SRG, said. “SRG Assure offers those residents receiving supportive care services a specialized focus, both during a hospital stay and after being discharged back to the community, all within a service-enriched, residential living environment.”
Hagerman said the Hospital Readmissions Reductions Program (HRRP), a Medicare value-based purchasing program, was created to improve healthcare for Americans by linking payment to quality measurements such as readmission rates for hospital care. The SRG Assure program encourages collaboration with healthcare providers to reduce readmission rates and obtain the best outcomes for patients.
The success of SRG Assure is that it is uniquely focused on the key factors that can contribute to someone’s likelihood of being readmitted to the hospital. Statistics demonstrate SRG’s success to date:
- With national 30-day readmission rates as high as 20% for those 65 and older, SRG percentages are less than 10% with an average resident age of 84. (With outcome tracking data for over three years, SRG’s 12-month rolling average as of June 2019 is 9.2%.)
- Further data shows 8.8% of SRG residents went to the ER in January 2019 as compared to the senior living industry national average of 14% (CDC, National Center for Health Statistics).
- In January 2019, 12.9% of SRG residents had a fall, compared to a national average of 22% for senior living residents.
SRG Assure works with hospitals, discharge planners and other key audiences to reduce hospital readmission rates by:
- providing up-to-date resident health information to serve as a baseline when the resident is admitted to the hospital
- coordinating care upon a resident’s discharge and return to the community
- implementing physician orders quickly and fully
- tracking prescription orders and ensuring that medications are being taken at the right time, in the right dose, in coordination with SRG’s medication management services
- coordinating residence modifications and onsite rehabilitations services as needed
- coordinating a follow-up appointment within seven days of discharge
- providing access to an RN 24/7 to help assess changes in conditions and to establish an action plan
- providing ongoing training and outcome data collection
Once a resident has transitioned back to the community during the recovery period, SRG’s Assure Program is further enhanced by, and residents benefit from, SRG’s ZEST program. ZEST is an evidence-based wellness program that stratifies risk and health outcomes to improve quality of life, minimize decline and maintain activities of daily living (ADL) function. The Zest program focuses on each resident’s individual challenges, strengths and goals to enable the infusion of personally satisfying and health-promoting activities in daily life.
The SRG Assure program is a continuation of SRG’s ongoing commitment to the highest standards of quality care and services. SRG senior living communities voluntarily participate in the Commission on Accreditation of Rehabilitative Facilities (CARF) to review and rate its communities. CARF is an independent, nonprofit organization that provides accreditation for health and human services. Similar to a Joint Commissions accreditation for hospitals, CARF accreditation demonstrates comprehensive conformance to the more than 1,500 elevated and internationally accepted standards that promote excellence.
“In 2004, SRG put its licensed communities to the test and voluntarily asked the Commission on Accreditation of Rehabilitation Facilities (CARF) to review and rate its level of care,” Hagerman said. “Not only were we one of the first nationwide providers to initiate the process, today we’re one of the few licensed providers to have all of our eligible assisted living communities accredited.”
